Accelerating Operational Readiness for Value-Based Care

A regional healthcare network was preparing to enter its first risk-based contract, but readiness gaps across departments were slowing progress. While leadership was aligned on the vision, teams lacked a unified approach to data tracking, performance accountability, and care coordination. Meetings often ended with more questions than clarity, and critical initiatives were delayed due to unclear ownership. With payer deadlines approaching and millions in potential revenue at stake, the organization turned to Chai Class Consulting to establish a structured readiness model one that could align leadership, simplify execution, and build confidence before contract launch.


We began with a 4D Framework assessment  Discover, Diagnose, Design, Deliver  to identify the disconnects between strategic intent and operational reality. Through interviews with department heads, we found three recurring barriers:

  1. Fragmented accountability between clinical, quality, and finance teams.

  2. Reactive workflows that weren’t built for continuous improvement.

  3. Limited visibility into performance metrics critical to value-based care.


Working alongside leadership, we co-created an Operational Readiness Roadmap  a 90-day action plan defining priorities, accountability owners, and success indicators for each function. We introduced weekly leadership alignment sessions to ensure progress tracking and decision agility. Next, our team helped design standardized workflows for data validation, care gap closure, and reporting accuracy making readiness measurable, not theoretical.


A simple but powerful “readiness dashboard” visualized performance progress for all stakeholders. Each department could instantly see where they stood on compliance, cost control, and quality measures.

Within 90 days, the organization achieved full readiness for its first value-based contract two months ahead of schedule. Leadership alignment improved dramatically, with 100% of departments completing assigned milestones on time. Operational turnaround times for reporting were reduced by 28%, and care gap closure performance increased by 22% in pilot sites.


Perhaps the most meaningful outcome was the shift in mindset. Leaders stopped “reporting on problems” and began proactively solving them together a foundational skill for success in risk-based care.


The readiness roadmap created during this engagement is now embedded as a standard pre-contract process across the organization, ensuring future scalability and accountability.
