Chai Class Consulting was founded to help healthcare organizations identify opportunities, create strategy, and execute the programs and changes they believe in, but struggle to implement.

With more than two decades of experience in healthcare strategy and operations,
Silvana Fischman has worked alongside provider groups, management services organizations, health systems, specialty networks, and dental support organizations to build the operational infrastructure that makes growth sustainable and performance measurable.

Silvana’s expertise spans clinical documentation improvement, risk adjustment, revenue cycle management, and value-based care program design. Her background
includes senior leadership at ChenMed, a nationally recognized value-based primary care organization serving Medicare Advantage populations, where she led initiatives that improved HCC capture accuracy, Stars (HEDIS), strengthened CDI programs, and aligned clinical and financial performance under value-based contracts. That clinical-financial fluency is what she brings to every engagement, whether the work is a full operational redesign, a KPI framework build, or a change management initiative.

As a Doctor of Dental Surgery trained internationally, Silvana also brings a rare clinical lens to dental support organizations navigating growth, multi-site standardization, and operational complexity. This firsthand understanding of the clinical environment, combined with her business credentials, allows her to engage DSO leadership at a depth that most operational consultants cannot match.

executive leadership has contributed to conversations across the U.S. healthcare
industry through speaking engagements, published work, and peer advisory
